We provide as much or as little assistance as you require to increase participants’ ability in integrating their existing NDIS supports, maintaining their development plan, making seamless transitions and actively transitioning into a new life stage.
Change can be difficult, especially when it involves new locations and circumstances. Integrity Disability Hub provides a wide range of assistance to assist with life stage transitions under this NDIS support category. The aim was to make this transition period as seamless as possible without interfering with the participant's comfort. We collaborate with the participant to provide a smooth transition and to assist the participant to settle into a new environment to develop a comfortable attitude.

As everyone's circumstances are unique, we will assist you in tailoring your NDIS plan to you, taking into account factors such as age, gender and family relationships.

We have a dedicated team of support workers who value our clients and guide them through support programs to train them to be more independent. We also have experienced support coordinators who link our clients with community access and other services tailored to their needs.
