Support Coordination Levels

Level 1: Basic Support Coordination

Provides guidance for understanding and utilising NDIS plans, including:

  • Understanding Your Plan: Assistance with budgets, funding management, and accessing required support.
  • Connecting with Services: Information on available supports, exercising choice, and connecting with providers.
  • Establishing Supports: Creating service agreements, understanding pricing limits, and identifying barriers.
  • Coaching and Reflection: Skill building, monitoring goals, and preparing for plan reviews.
  • NDIA Reporting: Progress updates, funding utilisation, and recommendations for future needs.

Level 2: Enhanced Support Coordination

Builds on Level 1, focusing on improving independence and community inclusion:

  • Designing Support Approaches: Identifying preferences, scheduling services, and evaluating options.
  • Building Capacity: Empowering participants to make decisions and pursue goals.
  • Crisis Planning: Developing plans for potential crises and navigating changes in circumstances.

Level 3: Complex Support Coordination

Builds on Level 2, providing comprehensive management for participants with complex needs:

  • Addressing Complex Barriers: Identifying and solving issues that hinder access to support.
  • Service Plan Development: Ensuring cohesive service delivery and facilitating communication among providers.
  • Crisis Management: Providing consistent support and coordination during emergencies or unexpected events.
