Managing Life Stages & the NDIS

We hope that by providing this support service, NDIS participants will gain confidence in connecting with a wide range of support services available in the community and health sector. We provide as much or as little assistance as you require to increase participants’ ability in integrating their existing NDIS supports, maintaining their development plan, making seamless transitions and actively transitioning into a new life stage.

What exactly is Managing Life Stages?

Change can be difficult, especially when it involves new locations and circumstances. Integrity Disability Hub provides a wide range of assistance to assist with life stage transitions under this NDIS support category. The aim was to make this transition period as seamless as possible without interfering with the participant's comfort. We collaborate with the participant to provide a smooth transition and to assist the participant to settle into a new environment to develop a comfortable attitude.

Planning your NDIS

We are committed to enhancing the quality of life for individuals with disabilities at Integrity Disability Hub because we think that everyone has the right to get the assistance they require. We cultivate partnerships with the local community, health experts, service providers and most importantly, you, to give the best possible outcomes to assist you in living a fulfilling life. We are a knowledgeable team of NDIS Plan Managers and Support Coordinators with a people-first mindset. As everyone's circumstances are unique, we will assist you in tailoring your NDIS plan to you, taking into account factors such as age, gender and family relationships.

Our social activities support services

  • Assisting with the development of daily skills such as cooking, menu planning, house cleaning, laundry and budget planning
  • Linking you to the community, leisure and cultural activities
  • Building skills to use local facilities and services such as banks, shops, public transport and recreational facilities
  • Supporting you to become more involved with others in the community, and to experience a variety of activities and relationships
  • Finding the right service e.g. In-home care, counselling and/or leisure
  • Assisting with referrals to health and social support professionals
  • Finding a job
  • Finding a place to live
  • Identifying eligible funding support
  • Assisting with funding applications
  • Working with family members or support networks to resolve issues
  • Assisting you in the transition to NDIS

 

We provide these services free of charge to qualifying clients and provide them priority access to our services:

  • Have severe or multiple disabilities
  • Geographic or social isolation
  • Lack of personal support networks
  • People who are not currently employed, in training or in a day service
  • People whose support arrangements are in jeopardy

 

Resources:WE HAVE A DEDICATED TEAM OF SUPPORT WORKERS WHO VALUE OUR CLIENTS AND GUIDE THEM THROUGH SUPPORT PROGRAMS TO TRAIN THEM TO BE MORE INDEPENDENT. WE ALSO HAVE EXPERIENCED SUPPORT COORDINATORS WHO LINK OUR CLIENTS WITH COMMUNITY ACCESS AND OTHER SERVICES TAILORED TO THEIR NEEDS.
